Care Coordination for families of children and youth with special needs. Cedar staff will work closely with the family and professionals to access needs and to develop a Shared Plan of Care.

At Three Cedar, we tailor an action plan to the goals you set for your child and family, and you’ll play a key role in making decisions about the plan. The Children's Neurodevelopment Center (CNDC) at Hasbro Children's Hospital provides evaluation and treatment of children with neurological, genetic, developmental, metabolic and behavioral disorders. The center incorporates multidisciplinary care with pediatric urology, orthopedics, neurosurgery and craniofacial programs.
The center is located on Prairie Avenue, in an area designed to be particularly spacious, comfortable and welcoming to children and their parents, who are essential partners in their children's care.

Services Provided
The ACT Now Clinic provides assessment and clinical triage of pediatric patients and their families, and allows for earlier access to specialty care. The clinic uses a process-oriented triage and care coordination model, including the development of clinically appropriate care plans for children with complex conditions. Clinical teams include a licensed clinician and a family service coordinator.
As is appropriate, families will be given the option to join our Three Cedar Family Center program for ongoing support and care management service.

Afia Center for Health & Wellness at 485 Plainfield St. in the Silver Lake neighborhood of Providence. Afia is a drop-in center that operates on Wednesdays and Thursdays 12-3 p.m. and offers hot meals; a food pantry that also includes access to personal care items; outings and group activities; and much more. New members must meet with APRI’s program coordinator to determine eligibility for services. There is no fee associated with the services offered through the Afia Center.

Children with Special Healthcare Needs includes: children who are eligible for Rhode Island Medical Assistance through Supplemental Security Income (SSI), Katie Beckett or Adoption Subsidy.) Children with Special Healthcare Needs (CSN) is Rhode Island's Medicaid managed care plan for Children with a disability or chronic condition and they are determined eligible for: Supplemental Security Income (SSI), Katie Beckett or Adoption Subsidy through the Department of Children, Youth, and Families (DCYF). When eligible for Medicaid, parents or guardians are then given a choice of enrollment in a participating health plan. Children who live in an institutional facility are excluded from health plan enrollment. In those situations, children will be enrolled in traditional Medicaid Fee-For-Service.
Children with Special Healthcare Needs may qualify for RIte Care under UnitedHealthcare Community Plan. The Department of Human Services will enroll Children with Special Healthcare Needs who are eligible for Medicaid.

Eligibility
Children under age 19 (up to 21 in certain cases) with behavior issues, juvenile delinquency or other problems that place them at risk of involvement with the Rhode Island Department of Children, Youth and Families (DCYF).
Available for families living in the West Urban Core or more specifically the communities of Providence and Cranston.
Services Provided
The Family Care Community Partnership (FCCP) serves families at-risk for involvement with the Rhode Island Department of Children Youth and Families.
FCCP services include referrals; triage; initial needs screening; in-person intake; resource information; referral and linkages to community resources; and high-fidelity wraparound services (which include crisis stabilization and are a type of family preservation service).
