Utah Department of Health Logo
The Portal's roles in the Pediatric Patient Summary/Medical Home Portal project are supported through the Utah Division of Medicaid and Health Finance's 2017 State Health Information Technology (HIT) Implementation Advance Planning Document-Update (IAPD-U), awarded 9/1/17 by the Centers for Medicare and Medicaid Services (CMS) and the Office of the National Coordinator for Health Information Technology (ONC) with funds allocated by Health Information Technology for Economic and Clinical Health (HITECH) Act of 2009, part of the American Recovery and Reinvestment Act (ARRA) of 2009.

The Utah Department of Health (UDOH)'s, Division of Family Health and Preparedness (UDOH), Maternal and Child Health Bureau (UDOH) provides support and collaboration in the Portal's ongoing development and maintenance of its directory of Utah providers of services for children and youth with special health care needs and their families.

The Nevada Division of Public and Behavioral Health, Nevada Title V Maternal and Child Health Program provides funding and collaboration to develop the Nevada Medical Home Portal and maintain its directory of local service providers for children and youth with special health care needs and their families.

Between 2005 and 2017, development of the Medical Home Portal was funded in part by the Health Resources and Services Administration (HRSA), Maternal and Child Health Bureau (MCHB) for its role in supporting the aims of grants H02MC00082, D70MCD4477, H6MMC11061, U22MC16508, and D70MC275500100, each awarded to the Utah Department of Health (UDOH).
