Agency-owned or operated facilities that provide an alternative living environment for adults with developmental disabilities, sensory impairments, physical disabilities, emotional disabilities, multiple disabilities or chronic illnesses who are in need of personal services, supervision and/or assistance essential for self-protection or sustaining the activities of daily living and consequently are unable to live with their own families or in a more independent setting.

Eligibility: Eligible for services under the Division of Developmental Disabilities. A Shared Living Arrangement (SLA) is a residential option for adults with Intellectual and/or Developmental Disabilities (I/DD) who reside in Rhode Island who are in need of 24 hour care.

Community Connections provider developing job skills
Provider Profiles: The Shared Living (SLA) model is based on carefully matching a person in need of support with a host family in a natural home, as opposed to an institutional environment. The person receiving support is an active participant in the process of selecting the host family. Services include having supports, meals and activities that are a good fit for them.
The host family is a paid contractor for their services.
