Please read these Terms of Use carefully before using the Service Providers Lists feature on the Medical Home Portal website. Your access to and use of the Lists feature is conditioned on your acceptance of these Terms. These Terms apply to all visitors, users, and others who access or use the Lists feature. By accessing or using the Lists feature you agree to be bound by these Terms. If you disagree with any part of the terms then you may not access the feature.

An email address is required to create an account on the Medical Home Portal and use the Lists feature. We will not sell or share your email address for any reason without your express permission. We will use it to contact you infrequently to ask for feedback and satisfaction.

You agree, for your protection and for validity reasons, to never share your password with another person. You also agree to never use another person's account for any reason. We highly recommend that you use a complex and unique password to prevent unauthorized access to your account and your lists.

After you register and log into the Portal, you will be asked to complete a “profile” that is intended to enable the Portal staff to better meet the needs of our users. You may provide as much or as little of the requested information as you prefer, with the exception of your email address.

Please note that, with each login, the Portal’s software places a cookie (a text file containing bits of information such as your username and password), in your browser's cache. This is only used to maintain your login status (logged in or out) and to recognize you when you return from the same computer and browser and auto-fill the username field in the login screen. The Portal software does not collect any data from your computer nor does it send any other form of information to your computer.

The Portal’s List feature allows you to create, edit, store, link to, print, share, and otherwise make available certain information, text, or other material ("Content") in lists you create. You are responsible for excluding any personal health information (PHI) or personally identifying information (PII) in the names of the lists or in comments within the lists.

We do not warrant the accuracy, completeness, or usefulness of any Content created or presented within Lists you create. This policy applies to member account information as well.

Portal Lists may include links to third-­party web sites or services that are not owned or controlled by the Medical Home Portal. The Medical Home Portal has no control over, and assumes no responsibility for, the content, privacy policies, or practices of any third party web sites or services. You acknowledge and agree that the Medical Home Portal shall not be responsible or liable, directly or indirectly, for any damage or loss caused or alleged to be caused by or in connection with use of or reliance on any such content, goods, or services available on or through any such web sites or services.

We reserve the right, at our sole discretion, to modify or replace these Terms at any time. If a revision is material or substantial, we will try to provide at least 30 days' notice prior to any new terms taking effect. What constitutes a material change will be determined at our sole discretion.
