Care Coordination in New Mexico

Care coordination for children with special health care needs often is complicated because there is no single point of entry into the multiple systems of care, and complex criteria frequently determine the availability of funding and services among public and private payers. Care coordination is a valuable tool to reduce disparities and improve access to health care and other social support services.

The New Mexico Care Coordination Consortium was established to be a source of information, resources, tools, expert advice, and peer learning and support for pediatric and family practice staff, managed care organizations, Medicaid, community-based family organizations, Title V staff and other service providers who focus on coordinating care for children, with an emphasis on those with chronic conditions and special health care needs.

The Consortium supports the work of the NM Title V Children with Special Health Care Needs program to promote access to a comprehensive Medical Home for all children. Complex and uncoordinated care is a contributing factor to poor health outcomes in the pediatric population of Children with Special Health Care Needs (CYSHCN).
