Primary care physician must complete and Fax or Email the Request for Services form to CANDO. 

Parents: Once your primary care physician has sent the Request for Services form, the request will be evaluated by our clinical staff and, if appropriate, we will contact you to schedule an appointment. If not, we will follow-up with your primary care physician with recommendations for a service that may better meet your needs. 

Fragile X Behavioral Health Clinic (all ages)
Jean Frazier, MD, David Cochran, MD, and other team members as needed, including Occupational Therapy, Speech and Language Therapy, Family Resource Specialist
•    Persons with a diagnosis of Fragile X Syndrome and behavioral challenges
•    Provide diagnostic and psychopharmacologic consultations
•    Provide ongoing psychopharmacologic treatment as indicated

Center for Autism and Neurodevelopmental Disorders (CANDO)

The Center for Autism and Neurodevelopmental Disorders (CANDO) is an outpatient clinic and collaborative effort between University of Massachusetts Memorial Medical Center and Department of Psychiatry at the University of Massachusetts Medical School that provides cutting edge services to patients in the Central Massachusetts area. CANDO offers a variety of services for youth and young adults with emotional and behavioral challenges and complex presentations. Link
