Festive News

Happy Holidays! The Medical Home Portal has some festive news to share.
The Medical Home Portal and the Utah Children’s Care Coordination Network will be supported for at least another 3 years by the Utah Department of Health. The Portal will also receive funding from Montana, Nevada, New Mexico, and Rhode Island. We appreciate and value our partnerships with these great states.
With the University of Utah’s Department of Biomedical Informatics, we have new federal support to develop an “app” for Epic and other EHRs that will enable clinical teams to create and maintain a Pediatric Patient Summary (PPS) for children with chronic conditions. The PPS will integrate access to the Portal’s information, guidelines, resources, and services directories. Save the Date: UCCCN Spring Learning Session

On March 23, 2018, the Utah Children's Care Coordination Network (UCCCN) will hold its Spring Learning Session from 8:30-11:30 am on the University of Utah campus to explore what families need from care coordination and their Medical Home/Neighborhood and to identify successful strategies and opportunities for improvement in how clinicians manage children with chronic and complex conditions. Attendees will include care coordinators from a number of pediatric practices in Utah and from the Utah Department of Health, care managers from insurers, patient/parent advocacy organization members, and others. We encourage you to plan to attend this no-cost event.
